Items to be Submitted with Applications

The following list of items are to submitted along with your application for a HABITAT home. Please use copies (not originals) of all the documents submitted.

INCOME:
▪ Most recent W-2 forms for the last two years (all working household members)
▪ Pay stubs for the most recent two (2) pay periods (all working household members)
▪ Copy of child support payment check, if applicable
▪ SSI Income–Copy of current award letter, if applicable
▪ HUD Income–Copy of award letter, if applicable

ASSETS:
▪ List of all assets if not listed on application
• Real estate
• CD’s
• Joint savings and checking accounts
• Stocks
• IRA’s and retirements accounts
• Savings Bonds
• Other assets
