CHECKLIST OF ITEMS TO BE SUBMITTED WITH THE APPLICATION  

(Complete this checklist and send it with your application.)

The following items are to be submitted along with your application for a HABITAT home.  Please use COPIES (not originals) of all the documents submitted.

 

  1. INCOME:
  • Most recent W-2 forms for the last two years (all working household members)
  •  Pay stubs for the most recent two pay periods (all working household members)
  • Copy of child support payment, if applicable
  • SSI Income – Copy of current award letter, if applicable
  • HUD Income – Copy of award letter, if applicable
  1. ASSETS: List of all assets if not listed on application, including:
  • Real Estate
  • Certificates of Deposit
  • Individual and/or Joint Savings and Checking Accounts – copies of two months statements for each account
  • Stocks
  • IRA’s and retirement accounts
  • Savings Bonds
  • Other Assets
  1. OTHER
  • Letter of Reference from the landlord
  • Copy of driver’s license or unexpired, authorized, government issued photo identification card (for both applicant and co-applicant)
